Activity 1: Create a care network and rehabilitation of the elderly People with mobility problems Ongkharak District Nakhon Nayok Province. - There is a network to care for elderly and those who have mobility problems in the community By people in the community themselves, 2 networks; Area 1) Ban Khmer Fang Tai Subdistrict Health Promoting Hospital Area 2) Khlong 14 Nuea Subdistrict Health Promoting Hospital - Carers for the elderly and those with mobility problems, have knowledge and good health behavior in order to have good health 100% (Areas 1 and 2) - Diabetic and hypertension patients, symptoms were controlled better by more than 40% and 60% respectively (Area 1). Elderly people and people with mobility problems are being rehabilitated. and take care of your health 100% correctly (Area 1 and 2) Homebound and bedridden patients decreased Reduce disability and burden on caregivers and communities Increase quality of life for patients (Area 1)
