3.8 Achieve universal health coverage, including financial risk protection, access to quality essential health-care services and access to safe, effective, quality and affordable essential medicines and vaccines for all 3.8.1 Coverage of essential health services (defined as the average coverage of essential services based on tracer interventions that include reproductive, maternal, newborn and child health, infectious diseases, non-communicable diseases and service capacity and access, among the general and the most disadvantaged population) 1. 100 % of Volunteers/Aged Care Leaders in the community areas, Khao Perm Subdistrict Area, Ban Na District, Nakhon Nayok Province, have been learned knowledge and practice skills to care for elderly people with chronic illnesses as planned.
2. 60% of Aged care volunteers in the Khao Perm Subdistrict Area can accurately transfer knowledge about the care of elderly people with chronic illnesses. (inevitable due to the covid-19 pandemic, discontinued practices)
3. 100 % of Elderly/elderly caregivers in areas are satisfied in volunteers/leaders of aged caregivers about the caring of elderly people with chronic illnesses.
4. 100% of Elderly people with chronic illnesses are educated by volunteers/leaders of elderly caregivers in preventing/controlling chronic diseases/health problems.
Caveat

Thailand has entered an ageing society since 2005. 18.43% (Institute of Population and Social Research, Mahidol University, 2019) and population estimates of Thailand The Office of the National Economic and Social Development Board (Pattanakarn) expects Thailand to enter an aged society with people aged 60 years and older equal to or more than 20% of the total population. And 10 years later or 2031, we will enter the super-aged society for the problems of the Elderly stay home alone and have a health problem in chronic illness. For the problem situation of the elderly of Nakhon Nayok province, was found that the number of elderly people is now rising rapidly, 23.28 per cent elderly population. The health problems of the elderly will also have chronic illnesses that correspond to the problems at the national level: There was 43.34 per cent hypertension and 18.57 per cent of diabetes. Almost have trouble accessing the health care service at hospital. The Volunteers/Aged Care Leaders in the community areas are important people who access and care for the basic health problem of the elderly in the area. Although, the Volunteers/Aged Care Leaders have trained and improve their skills. They can help to prevent the complication and promote good behavioural to the elderly in the area, and reducted the frequency to meet the doctor before the appointment and self-care monitoring by the elderly.

Impact

1. Creating volunteers/ leader cares to take care of the elderly who are suffering from chronic illnesses and access health with 4D wellbeing contributes to good quality of life and decrease the complications of chronic illness.
2. The elderly who are suffering from chronic illnesses are able to self-care monitoring before going to the hospital and decrease frequency to meet the doctor before the appointment
